What really caused the Note 7 recall? Battery, Charging System or Something else?

Does anyone have any reliable information about what the real issue was with the Note 7 that necessitated the recall?

Was it:

1. The battery.
2. The charging system.
3. Other.

I can only find really basic information about the issue. Samsung says it was a "battery cell issue" but that just doesn't make much sense to me. I'm asking because I thought that Lithium-Ion batteries did not experience thermal runaway at all. So either, Samsung used some other battery chemistry or I have a misconception about lithium batteries in general.
